What are the causes of insulin resistance?

In some cases it is an inherited tendency. But more likely it is a reaction to the lifetime pattern of eating foods high in fat, starch and sugar and a lack of exercise.

High blood pressure, high triglycerides and high cholesterol are usually corrected with weight loss and exercise. If you are on medication for these conditions, continue taking them, but get reevaluated as you lose weight.
